Don't try to extract lifetime annotations in various cases. Extracting lifetime annotations from template instantiations or implicitly defaulted functions doesn't make sense, as explained in a code comment. This change removes the test LifetimeElision_ImplicitlyDefaultedCtor because we shouldn't have been trying to extract lifetime annotations from an implicitly defaulted constructor in the first place.
