Use more efficient deduplication of `Object[]` in `NestedSetVisitor`. `VisitedState` now uses either: * `ConcurrentIdentitySet`, if concurrency is needed. * The recently added `VisitedArraySet`, if concurrency is not needed. In addition to being more efficient, this also avoids exposing internal `Object[]`s outside of the `nestedset` package.
