Misc rendering fixes to Stardoc 1. Include a "generated by Stardoc" header in all output markdown 2. Include a newline between link HTML and header lines. Certain markdown-to-HTML renderers do not behave nicely without this newline. Fixes https://github.com/bazelbuild/skydoc/issues/173 and https://github.com/bazelbuild/skydoc/issues/167 RELNOTES: None.
