Refactor `InstrumentationOutputFactory` so that client can create local and bep `InstrumentationOutput` without directly accessing the builder. For local instrumentation output, `append` and `internal` become two optional parameters in the `InstrumentationOutputFactory#createLocalInstrumentationOutput()` method.
